Membership Agreement T & C
FOR MONTHLY PAYMENT MEMBERSHIPS
During the initial period membership cannot be cancelled. After the initial period, payments will continue until cancelled (see “How to Cancel” below). Direct debits may be processed on or after the date due, but not before. Monthly membership fees are guaranteed not to increase during the initial term other than for the annual change to the consumer price index, which is assessed and applied if necessary within the first quarter of each calendar year. After the initial period the rate may be increased at any time by One Five One Corporate Health Club giving 30 days notice in writing by email. When payment(s) are missed, any overdue amount will be added to the next Direct Debit unless otherwise agreed upon with One Five One Health Club management. If at any time four or more payments are overdue, then all payments up to the end of the initial period become immediately due. MISSED PAYMENTS All costs associated with recovery of any missed payments will be added to any amounts due. This includes a $20 administration charge, and all collection agency costs. Amounts overdue for three months or more will be charged a 2% fee each month until paid in full. HOW TO CANCEL After the initial period, membership may be cancelled by (1) Informing One Five One Corporate Health Club in writing (email is sufficient), with 28 days notice (from the date of receipt), AND (2) receiving a letter (email) from One Corporate Health Club confirming this. Confirmation letters (emails) will be sent within seven days of receiving a cancellation request, and the cancellation will take effect 28 days after receiving the request. MEMBERSHIP TRANSFERS If your payments are up to date, and your membership is either within its initial term or you have paid for your membership in full, you are able to transfer your membership to anyone who has not been a member of One Five One Corporate Health Club within 6 months of the transfer date. The transfer fee is $20. If you are on a monthly payment membership, then the new member simply continues your payments for you. FREEZING MEMBERSHIPS A membership with up to date payments can be frozen at no charge only with accompanying medical certificate for a maximum of 3 months during the initial term. Medical certificates can be used once every 12 months. While membership is frozen, it does not reduce any remaining initial term. In all cases, freezing cannot be backdated, and takes effect once a freezing form is completed. Confirmation your membership has been frozen applies only when you receive a confirmation email from One Five One Corporate Health Club. GENERAL I am physically & mentally sound to proceed with exercise. One Five One Corporate Health Club is not liable for any loss of property or accidents, whether or not these are contributed to by One Five One Corporate Health Club, its staff, or members. Refunds are not given for unused membership. Membership may be cancelled by One Five One Corporate Health Club if a member’s behaviour interferes with the enjoyment of others (in such cases refunds are not given, and liability remains for any amounts still owing). One Five One Corporate Health Club reserves the right to cancel any membership. YOUR PRIVACY: Information collected will be only used by One Five One Corporate Health Club for marketing or fee collection purposes.
